Immediate implant placement on maxillary molar using sealing socket healing abutment (SSA): case reports

Abstract
There are clinical limitations on immediate implant placement on molar area due to discrepancy between fixture size and wide extraction socket. Sealing socket healing abutment (SSA) is healing abutment that mimics transmucosal profile of existing natural tooth using CAD/CAM technology. This case report presents a case of immediate implant placement on maxillary molar using SSA. A 74-year old female patient had horizontal root fracture on #16. Immediate implant placement using SSA was planned. Guide surgery was done and SSA was connected after bone graft on the gap defects. After 3 months, final prosthesis was delivered. After 6 months of surgery, 3D scan was done to confirm changes of the alveolar ridge. During the treatment, healing was uneventful without wound dehiscence or inflammation. There was no abnormal marginal bone loss or gingival recession after 6 months. Alveolar ridge dimension was well maintained compared to spontaneous healing of extraction socket. Immediate implant placement using SSA is a predictable procedure for molar area. SSA shows its advantage in clinical convienience and minimal invasiveness. SSA can be more useful and lead more predictable outcome with bone graft procedure.
